MBCT Self-referral form

Assessment in the AccEPT Clinic

After an initial referral has been made to the clinic by a GP or other Mental Health Professional, if it is appropriate patients will either be offered treatment or be contacted by telephone to check that the therapies we offer would be appropriate for them. MBCT treatment begins with an individual “orientation” session for all participants to further discuss the MBCT programme and what it involves.
