Exit questionnaire
It has come to our attention that you are leaving the University and we would be grateful if you could complete this questionnaire at your convenience. The purpose of this questionnaire is to enable the University to examine the reasons why people leave the institution and to identify any improvements in employment practices or staff services that could be made.
We would appreciate it if you could be as frank and specific as possible with your answers. Please note that your comments will not influence future references or re-employment with the University.
Should you wish to complete this with assistance from your manager or your HR Business Partner/Personnel Manager/Advisor please do not hesitate to contact them to arrange a convenient time. Similarly, should you wish to discuss a section in more detail, please contact your HR Business Partner/Personnel Manager/Advisor to arrange a confidential meeting.
Information will be recorded onto a database and used to produce regular monitoring information to help improve staff issues within the University. Your identity will, however, be kept confidential. However, should a matter be raised that is deemed as serious and needs immediate action then this will be drawn to the attention of your Head of College/Professional Service. Information may also be shared with other appropriate offices, such as Equality and Diversity. Prior to this happening, you will be informed by Human Resources of who will be contacted.
The questionnaire will also assist the University in maintaining accurate data on employment for report to the Higher Education Statistics Agency (see the Personal Data page for more information).
