Please click this link to access the Research Passport/ Honorary Contract Clinical Questionnaire. You will be taken to the OPAS-G2 portal where you will be asked to enter your email address.

You will be sent a code to use to log in. Please follow the instructions given via the portal. Please ensure you have all the information required about your immunisations and vaccination history as any details you enter in the form will not be saved if you come out of it.

Once you have completed and submitted the questionnaire, an Occupational Health Advisor will review it. Following this, a clearance report will be emailed to you.

It may be necessary for the advisor to arrange an appointment to discuss any disclosures further before sending the report.
